Depends on who you mean. Sometimes it’s because they’ve altered the character so much that George RR Martin requests they change the name, which was the case with Talisa/Jeyne Westerling and Vargo Hoat/Locke. Sometimes like with Asha aka Yara Greyjoy, it’s because they thought Asha and Osha were too similar-sounding and that people would get the names mixed up.
